Obtain Certified Birth & Death Records
Certified copies of recorded Ohio birth and Portage County death certificates are available for purchase at the Kent City Health Department.
Health departments with vital statistic offices in Ohio can issue certified birth certificates for recorded births occurring in the State of Ohio from 1909 to the present day, and certified death certificates for recorded deaths in the county of death from 1909 to the present day. Forms of personal identification are not needed for requested birth records.
The Kent City Health Department is not authorized to issue or fax uncertified copies of birth or death certificates. All sales are final upon issuance, no refunds allowed. Exchanges will be considered by request within 2 years of issuance.
Vital Statistics Will Be Closed Sept. 1-3, Reopen Sept. 7, 2021
The Kent City Health Department vital statistics office will be CLOSED Wednesday, September 1st through Friday September 3rd due to staffing shortages. The vital statistics office will reopen Tuesday, September 7th at 8:00 am. The health department is CLOSED Monday, September 6th in observance of Labor Day.
You may still submit orders however they will not start being fulfilled after September 7th, 2021.
For immediate birth certificate needs, please visit the Summit County Public Health website at https://www.scph.org/birth-death-records. (Orders for death certificates can only be submitted in the county where the death occurred.)

Most convenient method to request certified copies of Ohio Birth and Portage County Death records. Current delivery options are by regular *USPS First Class Untraceable Mail, *USPS Priority (1-3 day) Traceable Mail or by in-person pick up (normally same business day).
*Kent City Health Department is not responsible for delays with mail delivery.
(Newborn birth certificates orders will be mailed by regular USPS First Class Mail and can take up to 2 weeks to become available.)

Social Security Numbers on Death Certificates
Per Ohio Revised Code (ORC) 3705.23(5): For the first five years after a decedent’s death, a decedent’s social security number shall not be included on a certified copy of the decedent’s death certificate unless that information is specifically requested to be on the certified copy. Relationship restrictions apply and the acceptable proof is required; for details, view the Social Security Number Guidelines and Addendum Form (PDF), effective October 15, 2015.
